Kristofer Smith, MD is medical director of Northwell Health Solutions and leads the organization’s Office of Population Health Management. As senior vice president, he provides clinical leadership of all population health activities, including oversight of clinical integration with CareConnect, Northwell’s insurance company. Board certified in hospice and palliative care, Dr. Smith was previously an assistant professor and the assistant program director at the Mount Sinai Department of Internal Medicine. He joined Northwell to lead system-wide care continuum initiatives in the post-acute setting, as well as to direct the Advanced Illness Management program. Dr. Smith is an associate professor at Hofstra Northwell School of Medicine and has published a number of papers on advanced care models for the frail elderly and has been a co-investigator on a number of grants investigating the outcomes of high-intensity primary care programs.
