The National Home-Based Primary Care Learning Network, supported by funded by The John A. Hartford Foundation, has led efforts over the past few years to improve care delivery in HBPC by creating a national learning community of HBPC practices focused on care quality and quality improvement.  

We seek applications for our tenth wave of practices to join our Learning Network that has included 102 practices across 37 states.
