The mission of The National Home-Based Primary Care Learning Network is to advance the field of home-based medical care through a coordinated program of quality improvement and applied research.
The Learning Network’s vision is that homebound persons receive high-quality evidence-based medical care in their homes.
We aim to improve medical practice in the home and expand the evidence base for home-based medical care by fostering communities of best practice engaging in quality improvement, and conducting applied research.
